We are a state-certified training center that provides free Community Health Worker (CHW) training and work placement for people with lived experience.

Requirements

  • Willingness to follow the evidence-based program with fidelity
  • Commit to serving as a HUB CHW in your respective community (Bexar, Atascosa, Comal, Guadalupe, Medina, Hays, or Kerr County) for a minimum of 6 months
  • Receive direct referrals from the HUB
  • Pass a background check
  • Pass CHW certified training
  • Complete learning service hours

Eligibility

  • Working age 17 or above — we can train and mentor candidates starting at 17, but the State of Texas will not issue CHW certification until the candidate turns 18
  • A high school diploma is not required — if you don't have one, our training roadmap includes support to help you earn it
  • Legally authorized to work in the U.S.
  • Resident of Bexar County or the surrounding service area
  • Reliable transportation to serve clients in your neighborhood
  • Basic English proficiency (bilingual Spanish/English a plus)
  • Access to a computer and internet for training
  • Adherence to HIPAA, client confidentiality, and a professional code of conduct
  • Minimum attendance during training and completion within the program timeline
  • Post-training service commitment in your assigned neighborhood

  • Training is provided at no cost, but completion does not guarantee employment. Placement is contingent on certification, background check clearance, and partner agency openings.
  • Employment is offered through a partner Care Coordination Agency, not directly through the HUB.
  • State CHW certification is issued by the Texas Department of State Health Services (DSHS), not by the HUB.
  • Background checks are reviewed individually. A prior conviction is not an automatic disqualifier — we welcome applicants with lived experience.
  • Program participation is subject to funding availability and limited cohort sizes.
